Service pricing protocol

Based on initial laboratory testing, a medical history review (registration and/or medical records), and a consultation, patients will receive treatment. The treatment protocol may vary during the course of treatment, as additional services become necessary or listed services are not performed, based on a change in the patient's condition.


Professional service fees will be billed for after these services:

 

  1. Registration and/or medical record review

  2. Each office visit and physician consultation

  3. In-house, phone and/or e-mail follow-up
    consultation(s)

  4. Interpretation of test and management prior to or during conception cycles

  5. Interpretation of test and pregnancy management, including e-mail interpretation and management

  6. Supervision of home care services

  7. Letter for medical appeals and/or letters of medical necessity

We want to inform our patients that insurance companies often do not cover the following fees:

Initial Medical Review (depending on complexity)

Every other month monitoring charge for active patients
